Step 2: GATHER AND SIGN REQUIRED DOCUMENTS
Gather your medical records and get a medical clearance from your doctor. Carefully review, date and sign the Authorization of Release and Use of Medical Records and include it with the other documents you’ll need to submit.

Step 3: SEND YOUR DOCUMENTS
Send us a copy of your medical records and a signed authorization of release and use.

Step 4: PAY
Settle the admission fee.

Step 5: ORIENTATION AND START OF SERVICE
